MEDICAL RECORDS QUESTIONS

Obtaining a copy of your medical record is very simple.

To start the process, simply print out the Authorization Form, complete and sign the form, then fax or mail the form to us.

* Please be sure to complete the form in its entirity and sign. An incomplete or unsigned Authorization Form cannot be processed.

Once received, your request will be processed as soon as possible during business days (excludes weekends and holidays).

Arrangements may be made with Health Information Management Correspondence to pick up your copy, otherwise, your copy will be mailed to you.

We will fax to your doctor’s office or another hospital if it is required for patient care.

There is a 25¢ per page photocopying fee due before copy is released for personal access. An invoice may be mailed prior to the actual records being copied and mailed to you. There is no charge if sent to physicians for health care reasons.

We accept check or money order via mail made out to “Pacific Hospital of Long Beach” or in person, you may use cash. We are unable to accept credit card or ATM payments.

Authorization Form

If you are unable to print the above form, we can fax or mail you the Authorization Form for you to complete.

NOTE: A relative or spouse or friend cannot request a copy of your medical records, or pick records up on your behalf, unless they have a power of attorney over you for your healthcare decisions. A copy of the power of attorney for healthcare decisions must accompany the request.

Behavioral Health records are release only upon the professional judgement of the psychiatrist who cared for you while you were here.

The psychiatrist may deny you access to your health record if they believe in their professional judgement the information contained in your medical record may be determental to you or an informant.

Pacific Hospital of Long Beach medical records are kept for 10 years for adults and 25 years for mothers, newborns and minors.

SUBPOENA OR COURT ORDER QUESTIONS

Subpoena’s MUST be addressed to: “Pacific Hospital of Long Beach” only. No other subpoena’s will be accepted.

Subpoena’s addressed care of Pacific Hospital of Long Beach will NOT be accepted, for example, Dr. ABC c/o Pacific Hospital of Long Beach.

Subpoena’s addressed to Pacific Hospital of Long Beach in conjunction with someone else will not be accepted, for example, Pacific Hospital of Long Beach, Dr. XYZ and Dr. ABC.

It is perferred a separate subpoena be issued for Medical Records, Radiology Imaging, and Business Office records and be served directly to the department.

BIRTH CERTIFICATE QUESTIONS

Birth Certificates are prepared immediately after the birth of your child by the Admitting Department.

You may reach the Birth Certificate Registration personnel by calling (562) 997-2117.

The birth certificate is prepared and sent to the State of California. The hospital does not issue Birth Certificates.

If your baby is under a year, you may request a copy from the Long Beach Department of Health & Human Services, 2525 Grand Avenue, Room 201, Long Beach, CA 90815. It is recommended that you apply in person, complete the application and sign it in the presence of vital records staff and pay the $17.00 fee.

If you are unable to apply in person, you may request an application in writing. Send a letter with your name, your baby’s name, date and place of birth to the above address. You will need to return the notirized statement with your application and the fee (check or money order made to: Department of Health and Human Services) to the above address. If your child is over a year, you may request a copy from Los Angeles County Recorder, Attention: Birth Certificates, 12400 E. Imperial Highway, Room 1002, Norwalk, CA 90650.   You may call (562) 462-2167 or visit the Los Angeles County Recorder’s Web Site for more information.

Social Security number for your baby may be obtained by completing paperwork immediately after the birth of your child.  Social Security office will mail out your baby’s Social Security Card within 5-6 weeks.  The Birth Certificate Registration personnel will have this paperwork.

If you didn’t fill out paper work for a Social Security number when you deliver your baby you must go to the local Social Security office with a certified copy of your child’s Birth Certificate to apply.  If you prefer, you may access their web site at www.ssa.gov or call toll-free number: (800) 772-1213.

DEATH CERTIFICATE QUESTIONS

The hospital does not issue Death Certificates.

For a death within one year, you may request a copy in person or in writing from the Long Beach Department of Health & Human Services, 2525 Grand Avenue, Room 201, Long Beach, CA 90815.

If you are requesting in writing you will need enclude the deceased full name, date of death, place of death, number of copies you are requesting, the fee of $13.00 per Death Certificate (check or money order made to: Department of Health and Human Services), your name, your address, and your phone number to the above address.  Be sure to enclose a self pre-addressed stamped envelope.  A Mortuary may apply on your behalf.  Allow 20 working days excluding holiday’s to process your request.  You may call (562) 570-4307 for more information.

RADIOLOGY IMAGING QUESTIONS

If you are requesting films such as radiology images, MRI, or CT, please refer to the Radiology Imaging department.

If you are requesting tracings such as an echocardiogram (EKG), please refer to the Cardiopulmonary section of this web site.

To obtain a copy of these films or tracings, please print and complete a separate Authorization Form and send to Attn: Radiology Department or Attn: Cardiopulmonary Department: Authorization Form for Radiology or CardioPulmonary.
